Job Description:

The Intake Coordinator is a resource person for referral sources and works closely with other members of the team to ensure adequate coordination and transition occurs to home health services. As the Patient Care Coordinator, you will schedule the nursing, therapy, and aid visits.


  • Excellent customer service skills
  • Great communication skills
  • Extensive knowledge of the regulatory guidelines governing home health admissions
  • Minimum of 2 years experience in the health care industry
  • Minimum one year experience in home health
  • Understands the issues related to the delivery of home health services and be able to solve problems effectively
  • Possess knowledge of the Medicare guidelines governing home health agencies
  • Superior computer skills
