In order to help you assess the needs and best care arrangement for yourself or a loved one, one of our staff Geriatric Care Managers can visit the prospective client’s home.
We’ll inquire about your health, evaluate at the physical environment for potential risks and then recommend a care plan customized to your needs.
There is no obligation to receive care after an assessment and you will not be billed for the visit; it is a courtesy on our part to help ensure the safety of our valued neighbors.
