What Is the GUIDE Program?
The GUIDE Model — Guiding an Improved Dementia Experience — is a program created by the Centers for Medicare and Medicaid Services (CMS) to improve quality of life for people living with dementia while reducing the strain on the family members who care for them. CareGivers of America is proud to connect our clients and families across South Florida to this support through one of only a few hundred organizations selected by CMS to participate in this program.
If your loved one has Original Medicare Parts A and B and a confirmed diagnosis of Alzheimer’s disease or another form of dementia, they may qualify for free, structured support — including a dedicated Care Navigator, personalized care planning, caregiver education, and up to $2,500 per year in respite services. There is no cost to enroll, and your existing Medicare coverage stays exactly the same.
Dementia and the Families Who Battle It Every Day
Dementia is not a single disease. It is a term for a range of conditions — including Alzheimer’s disease, vascular dementia, and Lewy body dementia — that affect memory, thinking, behavior, and the ability to manage daily life. Alzheimer’s disease accounts for the majority of dementia cases and often progresses slowly, making early support planning especially important.
For families in South Florida, the weight of managing dementia care often falls on a spouse, an adult child, or a small group of relatives who are balancing their own lives alongside increasingly complex caregiving demands. The GUIDE dementia program was built specifically to ease that weight — and to make sure families are not left navigating it alone.
What the GUIDE Dementia Program Provides

Every family enrolled in the GUIDE dementia program receives a comprehensive home assessment and a personalized care plan developed by an interdisciplinary care team. From there, the program wraps a layer of coordinated, ongoing support around both the person with dementia and the people who love them.
You Are Not Facing This Alone — We Can Help
Dementia affects millions of families across the country — and in South Florida, where so many seniors choose to age at home, the need for structured support is significant. The GUIDE dementia program was created because the healthcare system recognized that families were carrying too much without enough help. CareGivers of America is proud to help connect our community to this resource.
