What Is the GUIDE Program?

The GUIDE Model — Guiding an Improved Dementia Experience — is a program created by the Centers for Medicare and Medicaid Services (CMS) to improve quality of life for people living with dementia while reducing the strain on the family members who care for them. CareGivers of America is proud to connect our clients and families across South Florida to this support through one of only a few hundred organizations selected by CMS to participate in this program.

If your loved one has Original Medicare Parts A and B and a confirmed diagnosis of Alzheimer’s disease or another form of dementia, they may qualify for free, structured support — including a dedicated Care Navigator, personalized care planning, caregiver education, and up to $2,500 per year in respite services. There is no cost to enroll, and your existing Medicare coverage stays exactly the same.

What the GUIDE Dementia Program Provides

Every family enrolled in the GUIDE dementia program receives a comprehensive home assessment and a personalized care plan developed by an interdisciplinary care team. From there, the program wraps a layer of coordinated, ongoing support around both the person with dementia and the people who love them.

Your Care Navigator is your consistent point of contact — a trained professional who coordinates care, connects your family with community resources, and is reachable 24 hours a day, seven days a week.

Family caregivers receive structured training in dementia care — including how to manage behavioral changes, communicate effectively, and reduce the emotional and physical toll of caregiving.

Your care team connects your family to local South Florida resources, including meal delivery, transportation, support groups, and specialist referrals — all coordinated for you.

An interdisciplinary team develops a comprehensive, person-centered care plan that addresses your loved one’s dementia symptoms, co-occurring conditions, and day-to-day safety needs.

Qualifying caregivers may access up to $2,500 per year in respite services — time away from caregiving, at no cost, through in-home care, adult day programs, or other approved settings.

Clinicians review and reconcile your loved one’s medications and provide guidance on maintaining consistent medication schedules — one of the most common daily challenges in dementia care at home.
