What Is the GUIDE Program?
The GUIDE Model — Guiding an Improved Dementia Experience — is a program created by the Centers for Medicare and Medicaid Services (CMS) to improve quality of life for people living with dementia while reducing the strain on the family members who care for them. CareGivers of America is proud to connect our clients and families across South Florida to this support through one of only a few hundred organizations selected by CMS to participate in this program.
If your loved one has Original Medicare Parts A and B and a confirmed diagnosis of Alzheimer’s disease or another form of dementia, they may qualify for free, structured support — including a dedicated Care Navigator, personalized care planning, caregiver education, and up to $2,500 per year in respite services. There is no cost to enroll, and your existing Medicare coverage stays exactly the same.
